Esteem Palliative Care

Our palliative care program is patient and family-centered care that optimizes quality of life by anticipating and treating symptoms that prevent unnecessary suffering. Palliative care throughout the span of illness addresses all-inclusive needs and facilitates patient independence, access to information and individual choice. The goal of Esteem House Call Physicians is to provide a strong palliative care program for Dallas/Fort Worth and surrounding communities. Our team is comprised of Physicians, Nurse Practitioners, Medical Social Workers, and other healthcare professionals who share the same goal of providing comfort treatment to those with serious chronic illnesses. Led by Dr. Arif Sattar, a Palliative Care Physician, our focus is on “well-being” and “quality of life.” Our team works closely with Hospital Palliative Care Programs in managing patient’s symptoms at home and minimizing the need for hospitalizations.

Palliative Care Philosophy and Delivery

The palliative team provides services in the luxury of your home, which includes personal care homes, assisted living facilities, and skilled nursing homes. The team, led by a physician, develops a care plan individualized to each patient’s specific needs. This care plan includes that the team:

  • Sees the patient every 30 days
  • Updates the care plan accordingly to manage symptoms
  • Collaborates with the patient, families, palliative and non-palliative health care providers
  • Recommends additional services when needed, such as home health, provider therapies, and hospice services

Patients and families are assisted throughout the course of illness with physical and emotional support, and you can continue to see your Physician Specialist as needed while on the palliative program. Help is available 7 days a week with 24 hour call support.
